Below you will find the links to the questionnaires and information to fill out in order for us to get the best possible results with the coaching we are undertaking.

 All the records are kept in password protected folders to protect client confidentiality.

NEW CLIENT QUESTIONNAIRE – This is self explanatory and get’s all the basic information for your records. It is pretty indepth so please take some time to go through in detail where required.

GASTROINTESTINAL ASSESSMENT – A healthy gut will be a game changer and this will tell us what could be going on right now within your gastrointestinal area.

BRAIN HEALTH SCREENING – This will allow me to see a good depth of where your health and neurological health is currently in order to build the right plan for us to move forward. It will open in a new window and automatically send me the results

METABOLIC HEALTH QUESTIONNAIRE – This is an in depth questionnaire that allows me to see more as to where we need to start with the support we give your body. I recommend right clicking, downloading to your laptop and using the latest version of Adobe Acrobat reader

Other Information

Here are a couple other things to help us get the best possible results and the best information down in order to create your plan.

Practice Better – You will receive an invite to Practice Better, this is the app I use to keep client information and notes of our sessions, you can track your food and moods here by using notes or photos alongside us uploading any test results too. It also has it’s own messaging service which is fully secure for us to keep our messages on.

Before Pictures – (Only if your goal is physique based) I know this isn’t just about transformation but I want you to send me before pictures, it helps with progression as your physique changes so takie front, side and back ones then email them over to ollie@ojayhealth.com , they get stored in a password protected file and won’t be used for marketing unless you agree once you have your awesome after picture and success story. 

Freestyle Libre 2 – A constant glucose meter if you wanted to test your glucose this way, a little more expensive but you don’t need to prick your fingers with this, just put it on the arm and scan every few hours whilst you’re using a sensor, each sensor lasts for 2 weeks. You won’t need one all the time, I recommend 2 weeks, then a few weeks off before trying again to see the difference.
